As a Case Manager, you will serve as an advocate, connector, and partner for program participants, helping them navigate services and build community supports that promote long-term success. You'll work as part of a multidisciplinary team focused on delivering person-centered, recovery-oriented care.

What You'll Do

As a Case Manager , you will:

• Participate as a member of a multidisciplinary team providing holistic, recovery-oriented services.
• Assist in developing and reviewing recovery plans in collaboration with participants, clinical staff, medical providers, and other stakeholders.
• Develop and maintain relationships with community agencies and service providers.
• Connect program participants with critical community-based resources and supports.
• Assist individuals in achieving recovery plan goals through advocacy, education, and service coordination.
• Work directly in the community within the program's service area.
• Transport program participants to appointments and community resources as needed.
• Complete assessments, documentation, and reports in accordance with organizational, funding, licensing, and accreditation requirements.
• Perform additional duties as assigned.

Qualifications

Required

• Two years of experience providing support to individuals involved in behavioral health, addiction recovery, intellectual and developmental disability services, homelessness services, or related human services programs.
• Strong interpersonal and communication skills.
• Intermediate computer skills.
• Ability to work independently in community-based settings.
• Ability to complete documentation accurately and timely.
• Subject to applicable background clearances and employment screenings.

Preferred

• Bachelor's Degree in Human Services or a related field.
• One year of Case Management experience.
• Experience working within behavioral health or community support programs.

Schedule

• Full-Time
• Community-Based Position
• Flexible schedule based on participant needs and program requirements
• Evening and weekend availability may be required
• Local travel throughout the service area required

Physical Requirements

• Ability to stand, sit, walk, climb stairs, reach, bend, kneel, stoop, crouch, and perform repetitive motions.
• Ability to lift up to 50 pounds occasionally.
• Ability to safely transport participants as needed.
• Ability to perform essential job functions with or without reasonable accommodation.
