About the Role We are seeking a dynamic and relationship-driven Clinical Liaison to support patient referrals and admissions to Schwab Rehabilitation Hospital. In this role, you’ll blend clinical expertise with outreach and coordination—building partnerships, evaluating patients, and ensuring seamless transitions into care. This is an ideal opportunity for a clinician who enjoys autonomy, collaboration, and making a direct impact on patient access to rehabilitation services. What You’ll Do Build and maintain strong relationships with referral sources (case managers, social workers, physicians, etc.) Conduct on‑site and remote patient assessments to determine rehab eligibility Coordinate admissions and ensure timely patient transitions Educate patients, families, and referral partners on acute rehabilitation services Manage multiple referrals and provide admission decisions within 24 hours Ensure accurate and compliant documentation and physician approvals Identify new referral opportunities and grow referral networks Collaborate with internal teams to achieve census goals What We’re Looking For Degree in Nursing, Social Work, PT, OT, Speech Therapy, or Athletic Training Active Illinois licensure required Experience in post‑acute care or clinical liaison role preferred Strong communication, multitasking, and decision‑making skills Ability to work independently and manage a fast‑paced workload Experience with EPIC or MEDITECH (preferred) Valid driver’s license Why Join Sinai Chicago?
