Continuously improve the patient experience while ensuring high-quality care. Evaluate, predict, and facilitate the trajectory of patient care. Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation. Participate in an in-depth orientation and onboarding program. Engage in the "Novice to Expert" program for professional development. Utilize resources from case management specialists and strong leadership support.
