Responsibilities
- Conduct initial and ongoing assessments of residents’ mobility, balance, strength, gait, and functional limitations.
- Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability, aligned with physician orders and facility policies.
- Deliver direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and the use of physical agents or modalities.
- Monitor resident progress and adjust care plans to meet therapy goals; participate in interdisciplinary care team meetings and contribute to discharge planning as appropriate.
- Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements.
- Accurately document treatment sessions, progress notes, and billing information in a timely manner; ensure compliance with state, federal, and facility documentation standards.
- Support quality assurance and performance improvement initiatives as part of the rehab team.

Requirements
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the state of practice (Missouri).
- Experience in SNF or geriatric rehabilitation is preferred.
- Strong clinical, communication, and organizational skills.
- Ability to work independently and as part of an interdisciplinary team.
- Comfortable working in a skilled nursing/long-term care setting.
