Job Summary As a member of a multidisciplinary health care team, the clinical pharmacist provides consistent clinical, consultative, research, and administrative pharmaceutical services for critical care, oncology/hematology, and other advanced practice areas. The role focuses on pediatric Hematology, Oncology, and Stem Cell Transplant services, and involves working alongside a multidisciplinary cancer care team to provide clinical and consultative pharmaceutical services. Prior experience in oncology or oncology focused residency is preferred. Essential Functions Identify differences between drugs as they relate to therapeutic efficacy in critical care and oncology/hematology. Apply knowledge of the physical and chemical properties of drugs to compute dosages, weigh and measure drugs, and mix and compound drugs for critical care and oncology/hematology. Apply pharmacodynamic and pharmacokinetic knowledge; anticipate and prevent chemical reactions; evaluate dosages and permissible concentrations. Work in an advanced practice area to promote and facilitate the adoption of best practices and innovations in service, technology, and operations. Assist with discharge medication coordination, prior authorizations, and provide discharge medication education to patients. Round with interdisciplinary teams in acute care areas to provide drug information, coordinate medications, and educate patients and families on a rotational basis. Provide in‑service education to pharmacy, nursing, medical residents, fellows, physicians, and pharmacists. Be present for planned bedside procedures to provide stat medications as needed. Consult with providers regarding patients in covered areas. Perform drug monitoring of patients and change orders per policy; participate in departmental initiatives and projects. Interpret prescriptions, dispense products, and call physicians about synergistic, antagonistic, or cumulative effects or questionable overdoses. Specify proper storage and protection of therapeutic agents per U.S. Pharmacopoeia standards. Stay current on new developments in the drug field and emerging patterns in pharmacy practice. Contribute to accredited teaching programs, training courses, and education of pharmacists, residents, interns, and students. Maintain records, prepare written and oral reports concerning patient care. Monitor medication therapy for safety and efficacy. Provide drug information to physicians, nurses, and other health care professionals. Serve in departmental and interdisciplinary committees. Supervise pharmacy technicians. Prepare sterile and non‑sterile products, check and dispense. Manage and control controlled substances. Act as primary responder to emergency events requiring a licensed pharmacist (e.g., Rapid Response, Code Blue Resuscitation). Minimum Qualifications Doctor of Pharmacy degree from a School of Pharmacy accredited by the American Council on Pharmaceutical Education. Completion of a PGY‑2 Residency program, or completion of a PGY‑1 Pharmacy Residency Pediatric program, or PGY‑1 plus 1 year of related hospital experience, or five (5) years of related hospital experience. Licensure/Certification: RQI Healthcare Provider Basic Life Support, or Basic Life Support by American Heart Association, Registered Pharmacist by California Board of Pharmacy, Pediatric Advanced Life Support by American Heart Association within 90 days of hire, or RQI Healthcare Provider Pediatric Advanced Life Support. Knowledge Knowledge of prescription and non‑prescription drugs, indications, efficacy, dosage, mechanisms of action, major contraindications, and side effects. Knowledge of medical terminology and abbreviations; pharmacology and pharmacological nomenclature; medical therapeutics, pharmacokinetics, and disease states. Knowledge of proper handling and storing of pharmaceutical supplies. Knowledge of influence of patient's age, sex, disease states, concurrent drug therapy, food, and diagnostic procedures on drug activity. Knowledge of physiology, anatomy, pathology, chemistry, and mathematics relevant to pharmacy practice. Knowledge of local, state, and federal regulatory requirements. Ability to apply judgment and make informed decisions. Ability to plan, organize, prioritize, and work independently to meet deadlines. Ability to foster effective working relationships and build consensus with other departments and vendors. Ability to communicate effectively and exercise tact in coordinating with staff, patients, and other professionals. Maintain familiarity with departmental information and technical systems. Ability to work with devices, information systems, and automated technical equipment used in pharmacy. Pay Range Compensation is based on the level and requirements of the role. Salary may also be determined by education, experience, skills, location, and internal equity alignment with market data. Typically new team members join at the minimum to mid salary range: $82.02 to $102.52 per hour.
