Hospice Referral Coordinator

Position: Hospice Referral Coordinator Join us and make a difference every day—because here, caring is more than what we do, it’s who we are. We are looking for a Hospice Referral Coordinator to serve as the first point of contact for patients, families, and referral partners. In this role, you will manage incoming referrals, ensuring a seamless onboarding experience onto our services. The Hospice Referral Coordinator plays a critical role in supporting timely access to care by efficiently reviewing referral information, verifying eligibility, and collaborating with clinical and operational teams. Location: Norwell, MA (Full-time, onsite, 40 hrs per week) Schedule: Monday - Friday 8:00 am - 5:00 pm Compensation: $23.00 – $28.00 per hour What You’ll Be Doing The ideal candidate is highly organized, customer-focused, and skilled in communication and problem‑solving. This position requires the ability to handle a high volume of referrals with accuracy, maintain compliance with regulatory standards, and build strong relationships with healthcare providers and internal stakeholders. Essential Functions Fields calls from referral sources; also retrieve e-faxes and/or e-mails for requests for service; obtain all necessary patient information. Screen potential referrals for appropriateness as established by Agency protocols. Coordinate with other staff to assure effective and efficient processing of referrals; i.e., forward necessary information and documentation to clinical staff and medical records as appropriate. Responsible for clerical accuracy and documentation of department-specific orders. Establish and maintain open lines of communication with referral sources to promote good community relations. Work with Billing Specialists on detailed insurance issues. Troubleshoot problems; assist and train staff members and make suggestions/recommendations for process improvements. Market Agency services and educate referral sources to support the goals of the Agency in order to grow. Perform duties necessary to maintain quality standards. Non‑Essential Functions Assure Agency compliance with all federal and state laws as well as adhering to Croí Health Corporate Compliance policy in the prevention of fraud and abuse on behalf of the Agency. Perform other Agency-related duties, as assigned. Desired Qualifications High school diploma required; AS in Secretarial Science preferred. Minimum of two years of office (clerical) related experience required, in a health care setting strongly preferred. Working knowledge of medical terminology. Prior experience as multi-disciplinary scheduler preferred. Strong proficiency in the English language. Proficient in Microsoft Word and Excel. Excellent computer and keyboarding skills required. Congenial and positive attitude. Able to function cooperatively as a team member. Highly organized, flexible and dependable. Able to work under pressure and meet deadlines and meet the agency's attendance requirements. Able to sit for prolonged periods (usually at computer).
