Registered Nurse Full time Salaried Bronx E147th, The Bronx, NY, US 22 days ago Requisition ID: 2430 Salary: $103,968.00 Annually Job Title: Registered Nurse Job Summary: The Bronx Nathaniel Assertive Community Treatment (ACT) Team is a multi-disciplinary, alternative-to-incarceration (ATI) program that provides evidence-based, mobile, community mental health treatment to promote the recovery and community integration of individuals age 18 and up with serious mental illnesses, high rates of co-occurring substance use disorders, high utilization of emergency/crisis services, and criminal legal involvement in the Bronx courts. The Bronx Nathaniel ACT Team is a unique and pioneer program that focuses on providing intensive outpatient treatment to individuals with serious mental illnesses so that they can maintain stability in the community and successfully complete their ATI mandates. The Registered Nurse provides medication management and administration, health assessment, counseling, and psychoeducation to promote recovery, wellness, and optimal health. The nurse provides at least 80% of services in the community within the framework of core ACT operating principles, professional nursing ethics, and regulatory guidelines. Shift Hours: This position is full-time Monday – Friday from 9:00 AM – 5:00 PM. Salary: The salary for this role is $103,968 per year. Location Address: 424 East 147th Street, 1st Floor, Bronx, NY 10455. Our offices are easily accessible by public transportation. Workplace Flexibility: Flexible - Staff have the flexibility to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work. What You Will Be Doing: Provides at least 80% of medication management and health services to consumers in the community, including but not limited to: medications administration (both oral and intramuscular), monitoring medications side effects and adherence, phlebotomy, coordination of consumers’ health care services, providing psychoeducation to consumers about health and medications, assessing consumers’ mental statuses, rotating on team’s after-hour emergency line Completes and signs progress notes, nursing assessments, incident reports, and other required documentation in accordance to the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations Collaborate with prescriber and Licensed Practical Nurse (LPN) on consumers’ medication management needs, health care services, maintaining consumers’ medication administration schedules Provide directions to LPN regarding medication management and administration services, health education, counseling, and psychoeducation services for clients Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers Participates in daily team meetings, clinical meetings, and monthly nurses meetings to collaborate and discuss clinical issues and clients’ needs with team members and nursing staff Use motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services Any other duties as required by Team Leader and senior clinical leadership.
