About the Role

We are seeking a compassionate and organized Bachelor of Social Worker to coordinate patient care and support positive health outcomes. This role focuses on assessing healthcare needs, developing and monitoring care plans, and coordinating services to ensure quality, timely, and efficient care.

The ideal candidate is patient-focused, collaborative, and committed to treating patients and families with dignity, respect, and compassion.

Key Responsibilities

• Coordinate patient care plans to ensure quality, timely service delivery, and medical necessity.
• Assess patient and family needs and identify appropriate services and resources.
• Develop, implement, monitor, and evaluate individualized care plans.
• Coordinate care with healthcare providers, patients, families, and support systems.
• Promote quality, efficiency, safety, and positive health outcomes.
• Maintain open and respectful communication with patients and families.
• Advocate for patients while protecting their dignity, privacy, and well-being.

Qualifications

• Bachelor’s degree in Social Work (BSW) or higher.
• Valid BSW credential in the state of Kansas.
• 1+ year of healthcare experience.
• 1+ year of experience in one or more of the following:
  • Case Management
  • Care Management
  • EHR Coordination
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively with patients, families, and healthcare professionals.
