Discharge Planner

Job Summary The candidate works closely with the case management team and assists in discharge planning services for patients admitted to Westchester Medical Center. They are a part of the multidisciplinary treatment team and be able to function independently on a professional level. The candidate must possess excellent oral and written skills, be organized and possess strong computer skills. Responsibilities Actively participate in multidisciplinary team rounds. Interacts with treatment team, patients, families, and community agencies in developing and arranging individualized discharge plans. Contribute in staff meetings, case conferences, case presentations and training activities. Maintain required documentation of work activities. In collaboration with the case manager and care coordinator, implement the discharge review program mandated by New York State Department of Health to facilitate issuance of IMM's and discharge notices to patients. Use computer applications such as spreadsheets, word processing, calendar, email and database software in performing work assignments. Access protected health information (PHI) in accordance with department assignments and guidelines defining levels of access. Perform other duties as assigned. Establish and maintain effective interpersonal relationships with patients and treatment team members. Physical ability to perform duties assigned. Qualifications / Requirements Experience: 1 year experience providing behavioral health services is preferred. Education: Associate’s degree, required or equivalent combination of experience and education may be substituted for degree requirement. Licenses / Certifications: (none listed) Other: Excellent written and communication skills. Commitment to enhancing cultural competency. Ability to prioritize and work in a fast-paced environment. Knowledge of Excel, Word, Outlook. Special Requirements: Knowledge of medical and pharmacology terminology.
