Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 08/03/2026 Duration: 13 weeks 36 hours per week Shift: 12 hours Employment Type: Travel ID: 63855317 Shift: Mon- Fri Description: Provide physical therapy services within the legal scope of practice for the state of practice, following all licensure requirements Maintain valid licensure and certifications as required by state and federal regulations Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare, state, and CareM standards Develop, implement, and update individualized plans of care aligned with physician orders and patient needs Deliver skilled interventions and maintenance therapy programs as permitted by state defined scope of practice Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes Appropriately apply ICD-10 coding for accurate and compliant documentation Present in-service programs periodically to agency staff and contribute to staff education initiatives Actively engage in Quality Assessment and Performance Improvement (QAPI) activities, supporting continuous compliance and clinical excellenceOther Duties: Provide care and services in accordance with infection control, safety, and risk prevention protocols Accurately document clinical interventions, care coordination, and communication in a timely manner Participate in interdisciplinary meetings, case reviews, and continuing education opportunities Maintain compliance with HIPAA and all company privacy and confidentiality standards Represent the organization positively and professionally in patients homes and community settings Work independently with minimal daily oversight and demonstrate initiative in completing responsibilities Provide courteous, timely, and professional customer service to patients, families, and partners Exhibit adaptability to changing responsibilities and flexibility in completing assignments Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms Adhere to all organizational policies, including job descriptions, mission, and the Employee Handbook Communicate and interact professionally and respectfully with others to support team goals Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the companyQualifications Graduate of an accredited physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation Currently licensed as a Physical Therapist in the state of practice Two years of clinical experience preferred; home health experience a plus but not required Ability to exercise sound clinical judgment and work independently in varied patient care settings Strong communication and interpersonal skills with sensitivity to diverse backgrounds Basic computer proficiency; willingness to learn EMR systems and OASIS documentation Current CPR certification Commitment to compliance with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies Reliable transportation, valid driver s license, and auto insurance for home visits Additional Information:***Home health patients to be seen within a 45 mile radius of Albuquerque area (s/a Alb, Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, & Belen) ***Rates posted are all-inclusive and will NOT include additional mileage reimbursement Position Urgency:Normal Billing string = Medefis Guaranteed Hours: 30 Contract Weeks:91 Prime Staffing Job ID #37397860.
