Field-Based Home Health RN Case Manager

H. T. PROF Group is seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and surrounding Tarrant County. This full-time position requires a clinically strong RN with recent home health case management experience, excellent assessment and documentation skills, and the ability to manage an assigned patient caseload independently in the field. You will travel locally to patients' homes, coordinate with physicians, families, and a multidisciplinary team, #J-18808-Ljbffr H. T. PROF Group
