PRN Delegating Nurse Case Manager

Compass, Inc. a progressive, non-profit organization that supports people with developmental and intellectual disabilities is seeking to hire a PRN RN Delegating Nurse Case Manager. This position supports Compass in the care and development of people served in residential programs and community settings. Working as part of a team, the PRN Delegating Nurse Case Manager helps coordinate health care, provide nursing support, and promote individual well-being. This role provides nursing coverage when regular nurses are on leave and may be assigned to support specific individuals, perform delegation duties, and assist with special nursing projects as needed. Primary responsibilities include coordinating health care for people in residential programs, training staff, and providing direct nursing support when appropriate.
While performing this job, the employee is regularly required to sit, talk, and hear. The employee is also required to stand and walk and must occasionally lift or move more than 50 pounds.
The position is performed in an office environment with a moderate noise level. Compensation- $45 an hour .
Essential Duties and Responsibilities
The following statements describe the general nature and scope of work performed in this position. They are not intended to be a complete list of all responsibilities, duties, or required skills. Other duties may be assigned.Care Planning and Clinical Oversight
- Review each individual’s nursing care plan and develop plans when needed.
- Ensure nursing plans of care are implemented appropriately.
- Observe and assess individuals regularly, documenting relevant findings in 45-day reviews and nursing notes.
- Review clinical documentation for accuracy, thoroughness, and clarity.
- Assist staff in maintaining medical charts in accordance with Compass policies and procedures.
Nursing Support and Delegation
- Delegate health care duties to direct support staff as needed, including medication administration.
- Administer medications and document them on the Medication Administration Record when needed.
- Provide therapeutic treatments that require a licensed practitioner.
- Train staff to provide routine clinical care, including proper positioning, safe lifting procedures, skin care, and vital signs.
- Assist with medical appointments and provide support during medical emergencies as needed.
Coordination and Communication
- Research health care resources and facilitate access for staff and individuals.
- Communicate with insurance companies to support access to and continuity of care.
- Assist individuals with payment-related issues, including purchase orders and payment disputes.
- Communicate with families to promote teamwork and understanding regarding clinical issues.
- Serve as a liaison to pharmacies and medical professionals as needed.
- Communicate respectfully and positively with staff, family members, professionals, individuals, and community members.
Individual Support and Wellness
- Promote a neat and clean appearance for individuals through staff training.
- Provide training and support to help individuals increase independence in managing their health.
- Encourage staff to support individual independence with activities of daily living and leisure activities in the home and community.
- Encourage exercise for both staff and individuals to support optimal health.
- Promote family involvement through effective communication and positive interactions.
- Support healthy nutrition for individuals at home and in the community

Qualifications

Requires completion of a registered nursing program, including a Bachelor of Science in Nursing (BSN), Associate Degree in Nursing (ADN), or diploma from an approved or accredited nursing program, along with successful completion of the national licensing examination.- At least 3–5 years of related experience working with children or adults in a caregiving setting.
- Prior experience navigating health care resources and insurance systems preferred.
- Supervisory experience preferred.
Language and Communication Skills
Must be able to read and understand simple instructions, short correspondence, and memos; write simple correspondence; and present information effectively in one-on-one and small-group settings with customers, clients, and other employees.Mathematical Skills
Must be able to add, subtract, multiply, and divide using basic numbers, including 10s and 100s, and apply these calculations to units of American money, weight, volume, and distance.Reasoning Ability
Must be able to apply common-sense understanding to follow detailed but straightforward written or oral instructions and resolve basic problems involving a few concrete variables in standardized situations.Computer Skills
Must be able to perform basic computer tasks using Microsoft Word, Microsoft Excel, Outlook, and other routine office systems.Certificates, Licenses, and Registrations
Must be a Maryland Registered Nurse with current CPR certification. Additional related training and certifications are required as mandated by DDA standards, COMAR regulations, and other applicable regulatory standards.Other Skills and Abilities
- Basic knowledge of COMAR standards and state regulations.
- Ability to interpret IP, IEP, and BP needs as they relate to individual care.
- Valid state driver’s license with a good driving record preferred, subject to approval.
- Ability to pass Compass background check and medical screening requirements.
