Description: The Care Manager will utilize a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet patients' health needs. The role involves using effective communication and available resources to promote quality, cost-effective outcomes for 18-22 patients. The Care Manager will address patient needs, manage the length of stay, and promote efficient utilization of resources. This position requires determining medical necessity and appropriateness of admission through screening processes. Extensive background and knowledge of current patterns of practice and acute inpatient care methods are essential. Candidates should have a solid understanding of physical and mental impairment assessments, illness characteristics, and expected treatment plans. Familiarity with benefits, plan contracts, eligibility issues, and requirements is essential.
