Clinical Pharmacist

Department: Inpatient Pharmacy Services Schedule: Full‑Time, Primarily Day Shift (Every 4th Weekend) Reports To: Pharmacy Clinical Coordinator, Director of Pharmacy Setting: Large Acute‑Care Hospital Position Summary The Clinical Pharmacist provides advanced, patient‑centered pharmaceutical care in a large, high‑acuity hospital environment. This role is responsible for daily interdisciplinary rounding, optimizing medication therapy, and ensuring safe, evidence‑based practice through established automatic renal dosing and anticoagulation adjustment protocols. The pharmacist collaborates closely with physicians, nurses, and allied health professionals to improve clinical outcomes and support the hospital’s mission of high‑quality patient care. Key Responsibilities Clinical Practice Participate in daily interdisciplinary rounds with medicine, surgery, ICU, or specialty teams. Perform comprehensive medication therapy management, including: Renal dose adjustments using hospital‑approved automatic protocols. Total parenteral nutrition (TPN) evaluation and support. Therapeutic drug monitoring and lab interpretation. Identify and resolve medication‑related problems, drug interactions, duplications, and contraindications. Document all clinical interventions and recommendations in the electronic health record (EHR). Provide drug information and evidence‑based recommendations to prescribers and nursing staff. Support antimicrobial stewardship initiatives as applicable. Operational Responsibilities Verify medication orders for accuracy, safety, and clinical appropriateness. Oversee sterile and non‑sterile compounding in accordance with USP / . Support automated dispensing cabinet (ADC) workflows and medication distribution processes. Serve as a resource for nursing and medical staff regarding medication preparation, administration, and monitoring. Day shift with consistent daytime hours. Holiday rotation as assigned. Occasional cross‑coverage for other clinical areas depending on departmental needs. Qualifications Required Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy from an ACPE‑accredited institution. Active pharmacist license (or eligibility) in the state of practice. Strong clinical knowledge base and ability to function independently in a fast‑paced environment. Proficiency with electronic health records and clinical decision support tools. Preferred Completion of an ASHP‑accredited PGY1 Pharmacy Residency. PGY2 residency or equivalent experience in internal medicine, critical care, or another clinical specialty. Board certification (e.g., BCPS) is highly desirable.
