Home-Based Occupational Therapist for Elder Independence
Senior Care Partners P.A.C.E.
Senior Care Partners PACE is seeking an Occupational Therapist to join our interdisciplinary team and help participants live independently at home and in the community. You will assess, plan, and provide customized therapy to improve ADLs and IADLs while collaborating with physicians, nurses, and other therapists. You’ll focus on person-centered care, environmental adaptations, and strategies to maximize safety and independence, delivering therapy in participants’ homes and coordinating with the #J-18808-Ljbffr
