Hospice RN Care Coordinator — Home-Visit Advocate

Chapters Health System is seeking an RN Care Coordinator to assess patient/family needs, coordinate the Plan of Care with the IDG, and provide palliative and supportive care in the patient/family unit. The role requires an active RN license, at least 1 year in hospice or hospital settings, and experience with EMR/EHR systems.
