Document Specialist
Quality Private Duty Care
Responsible for quality control of charting and audits. Qualifications An LPN or RN currently licensed by the state in which employed by the agency. At least one year of experience in home health care. Have good working knowledge of chart audits, Medicare and Medicaid guidelines as related to home health care. Good working knowledge of chart assessments, Medicare/Medicaid billing process, home health and private duty charting. Key Responsibilities Reviews OASIS, 485, Plan of Care on admission and discharge for accuracy and completeness. Sends correspondence to the visiting staff as required for corrections. Review nursing visit report, supplemental orders, home health aide visit report, and special discipline notes daily. Enters timely 485 into HHAMIS, transcribes OASIS data into HAVEN, locks data, and maintains log of all data entry. Transcribes, generates, and mails 485 data in a timely manner. Reviews and/or updates medication profile in HHAMIS as indicated via supplemental order. Audit admission, discharge, and recerts in a timely manner using the approved audit tool. Keeps Director and Team Leader informed of findings on a daily basis when necessary. Keeps ongoing contact with patient care staff and assists with audit orientation as assigned. Assists with compilation of information for PI Utilization Review on a quarterly basis or as scheduled. Organizes, prioritizes and executes work assignments effectively and efficiently. Receives instructions and keeps ongoing contact with Team Leaders and Director of Patient Services in assigned offices. Attends case conferences, inservices, seminars as scheduled, and other meetings as assigned by Director. Makes patient visits if requested. Maintains confidentiality of information related to business practices, activities, and personnel. Maintains a professional attitude when working with the staff, fostering the team philosophy and team environment for all personnel. Does not allow personal affairs to interfere with scheduled work time. Maintains a professional appearance at all times. Provides proper notification and/or advanced notice of absence or tardiness without abuse. Participates willingly with special projects and/or overtime work when requested.
