Position Details

• Location: Prior Lake, MN (55372)
• Hours: * Day HCN accompanying client to school
  • Days Sundays through Wednesdays & Every other Friday 9- or 10- hour shifts
  • Overnights Every Monday & Wednesday & Every other Friday & Saturday 10-hour shifts.
• Client Specifics: 10-year old with trach, vent, and a gj-tube

• Location: Prior Lake, MN (55372)
• Hours:
  • Days Monday through Thursdays 10 hour shifts
• Client Specifics: 15-year old with trach, vent, and a g-tube

What You'll Do

• Provide skilled nursing care in home and school settings
• Assess and monitor your patient's condition and communicate changes with the care team
• Administer medications, treatments, and prescribed therapies
• Provide trach, ventilator, airway, and G-tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment

Earn More for Select Shifts

Your flexibility makes a difference, and we believe it should be rewarded.

• Weekday Nights (Mon–Fri): +$1/hour
• Weekend Nights (Sat–Sun): +$2/hour
• Weekend Days (Sat–Sun): +$1/hour

Pay Range

You bring the skill and compassion that make one-on-one pediatric care possible. We’re committed to recognizing the experience and expertise you bring to our families.

• LPN: $25.00 – $31.00/hour
• RN: $35.00 – $41.00/hour

Starting pay is based on license, relevant experience, and qualifications.
