Care Manager Assistant - LPN

The Care Manager Assistant - LPN supports patient care coordination by monitoring, evaluating, and promoting quality services in compliance with laws and standards. Qualifications include a nursing diploma (associate’s or bachelor’s) active LPN license preferred clinical experience in post-acute care case management utilization review discharge planning Key responsibilities clear communication organizational efficiency decision-making adherence to policies maintaining confidentiality Essential functions utilizing computer systems liaising with external contacts training staff and families implementing care standards delivering excellent customer service assisting in patient evacuation functioning effectively within a team The role requires professionalism, integrity, good judgment, physical activity, and proficiency in English. #J-18808-Ljbffr Azza HealthCare Agency
