RN Clinical Case Manager

Job Overview RN Clinical Case Manager (Home Care) Responsibilities Supervise all home‑care personnel as assigned. Enforce and demonstrate compliance with State and Federal regulations. Coordinate the implementation of each patient’s Plan of Care through effective communication. Instruct, train, and re‑educate staff on clinical skills and procedures when necessary. Provide direct supervision of field LVNs, LPNs, and RNs per State guidelines. Offer support, guidance, and education to patients, families, and caregivers to maintain optimal physical and emotional well‑being. Communicate all occurrences and other pertinent information to the Clinical Supervisor/Director of Clinical Services. Report any observed or suspected child/adult abuse, neglect, or exploitation to proper authorities as mandated. Collaborate with Clinical Supervisor and Director of Clinical Services to resolve client/clinical concerns in a timely manner. Develop and implement treatment plans for each patient under physician direction. Complete all clinical documentation promptly, including admissions, recertifications, and resumptions of care, ensuring all requirements are met per agency policies and State regulations. Review documentation to verify completeness and alignment with payer source requirements for reimbursement. Interview and evaluate potential clinical field staff to ensure competency and promote safe patient care. Participate in continuing quality improvement, reporting incidents, injuries, infections, and risk‑management components. Engage in agency‑sponsored continuing education, in‑services, and conferences to maintain clinical competency. Support and execute the company’s mission, ethics, and goals. Represent the company positively and professionally in both the workplace and community. Adhere to the dress code, maintain a clean and neat appearance, and report to work on time. Follow all company policies and procedures as outlined in the Employee Handbook, Employee Agreement, or executive communications. Make patient and personnel assignments. Coordinate patient care and referrals. Ensure patient needs are continually assessed. Ensure development, implementation, and updating of individualized care plans. Qualifications Current state License as a Registered Nurse; BSN preferred. Minimum two years of nursing experience in a Home Health or Acute setting preferred. Demonstrated leadership capabilities with strong supervisory and interpersonal skills. Detail oriented, flexible, with strong organizational and time‑management skills. Knowledge of cost management for clinical services. Current CPR card. Current health certificate as applicable. Current state driver’s license and automobile insurance.
