Physical Therapist

one-on-one physical therapy to older adults in private residences, senior living, and community settings Perform evaluations, develop personalized treatment plans, and deliver evidence-based therapy Support patient mobility, independence, and wellness through therapy sessions Document accurately using EMR systems in compliance with regulations Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists Educate patients, caregivers, and staff to promote safety and aging in place Utilize telehealth and virtual care to enhance access and flexibility Role & Responsibilities one-on-one physical therapy to older adults in private residences, senior living, and community settings Perform evaluations, develop personalized treatment plans, and deliver evidence-based therapy Support patient mobility, independence, and wellness through therapy sessions Document accurately using EMR systems in compliance with regulations Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists Educate patients, caregivers, and staff to promote safety and aging in place Utilize telehealth and virtual care to enhance access and flexibility Qualifications & Skills Valid Physical Therapy license or eligibility in practice state(s) Degree from an accredited PT program Proficiency in computer use and EMR documentation Current CPR certification Key Duties & Physical Requirements Exert medium effort, including lifting up to 50 lbs occasionally Perform CPR and assist with patient transfers and mobility Reposition patients, stand/walk for extended periods, climb stairs Drive to patient locations and wear PPE as needed Benefits & Opportunities Offerings include competitive pay, medical/dental/vision insurance, 401(k), continuing education, certification reimbursement, career advancement, and access to advanced technology.
