Physical Therapy Assistant - Travel Contract

Minimum Qualifications

Associate degree in Physical Therapy from an accredited educational program. Active State License is required to start the assignment. BLS Certification may be required from AHA or ARC. Must have at least 12 months of Home Health experience within the past 3 years (client requirement). Experience with HCHB (HomeCare HomeBase) electronic medical record is required (client requirement). Previous experience providing care in a home health setting is required. Preferred: additional certifications or training in geriatric or orthopedic home health care.
