Description

• Conduct comprehensive evaluations of patients in inpatient and outpatient settings, tailoring personalized treatment plans
• Implement physical therapy strategies focusing on mobility, strength, balance, and functional recovery
• Work collaboratively with nurses, doctors, case managers, and other rehabilitation specialists to optimize patient outcomes
• Guide patients and family members on therapeutic objectives, discharge protocols, and home exercise routines
• Document patient interactions, treatment plans, and progress noting compliance with facility as well as regulatory demands
• Engage actively in interdisciplinary conferences and care team rounds to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• Preferred: CPR certification
