Candidate Requirements

Board Certified in Hematology / Oncology REQUIRED Completion of Hematology/Oncology Fellowship REQUIRED Active Kentucky Medical License Highly Preferred BLS & ACLS Certifications REQUIRED Experience with both outpatient and inpatient Hem/Onc care REQUIRED Ability to perform Intrathecal Chemotherapy REQUIRED Experience with Intracavitarian Chemotherapy Highly Preferred COVID Vaccination REQUIRED Self-Query NPDB report (dated within 30 days of submission) REQUIRED
