SAFY is seeking a full time Outpatient Case Manager for our Lima Behavioral Health division in Allen County.
As an Outpatient Case Manager, you will be responsible for providing services to clients referred into the outpatient mental health program. The scope of services provided by this position utilizes a broad array of intervention techniques required to effectively address client needs. What You'll Do
- Provide behavior management, social competency, problem identification and resolution, effective communication, self-sufficiency, and life goals to youth, families and individuals in service.
- Direct counseling and/or teaching services to assigned individuals and families utilizing a broad array of intervention techniques required to effectively address client needs.
- Provide parent education training and psychosocial interventions.
- Provide intervention techniques in the outpatient mental health program including behavior management, crisis management, social connection, resource support, and separation. These services are provided through in-home, video-conferencing and over the phone interactions, teaching, and guidance.
- Responsible for completion and accuracy of assessments identified (i.e., Risk Assessment, and Suicide/Homicidal Assessment).
- Work as a member of a team to develop, assign, and implement services to children and families with quarterly treatment plans and safety plans including explicit goals.
- Must meet weekly utilization standards as required by supervisor.
- Monitor and document treatment plan outcomes on a quarterly basis.
- Report via appropriate documentation all special incidents directly to the supervisor as well as referring agencies as defined in the contract/agreement.
- Timely completion of recorded services and progress notes.
- Participate in the 24-hour crisis intervention as assigned.
- Assist families in identifying and using community resources to successfully meet their needs. Maintain accurate up-to-date documentation as required.
- Establish contact and maintain professional communications with referral sources as required by program design.
- Establish and maintain a positive working relationship with all levels of SAFY staff by participating in joint meetings, and problem identification and resolution.
- Establish and maintain a supportive and positive working relationship with youth, foster parents, referring agencies, and other SAFY stake holders.
- Attend relevant in-house and external seminars, trainings, conferences and workshops to develop and enhance knowledge in the designated area as well as achieve relevant education hours per year to enhance professional and job-related skills.
- Attends required meetings within division, and central office (team and/or community partners).
- Participate in assigned committees and chairs if so appointed. If appointed, maintains liaisons with state and national organizations.
- Meets regularly with supervisor to discuss issues and performance.
- Submit weekly/monthly/quarterly reports to immediate supervisor.
- Sensitive and responsive to the cultural differences of staff, SAFY youth, foster parents, referral sources and the public.
- May be on call 24 hours or on a rotating basis to address crisis with clients.

Qualifications
- Bachelor's degree in the field of social work, counseling, psychology, sociology, child and family studies, or other approved related field of human services.
- LSW, LPC or license eligible preferred.
- Three years of relevant experience with above listed degrees, experience in child welfare preferred, OR relevant experience in working with children in general.
- Must have a valid driver's license, acceptable driving record and auto insurance coverage to meet travel requirements; may require transporting youth if needed.
- Targeted hiring range: $44,000 to $53,000
- Compensation is commensurate with transferrable experience, education and licensure of candidate, location of the position, along with internal equity and budgeted amount for the role.
