Registered Nurse - Family Medicine

Join Our Team as a Registered Nurse! TriHealth seeks a Registered Nurse to provide nursing services in Family Medicine. This role focuses on assessment, care planning, and interdisciplinary collaboration. Location: TriHealth Family Medicine – 8240 NORTHCREEK KWOOD Work Schedule: Full Time (80 hours bi-weekly), Day Shift, Weekend rotation. Job Overview This position provides nursing services, either directly or indirectly, to participants in a specific program, specialty department, or practice. The RN performs initial assessments and regular re-assessments, focusing on prevention, early detection, acute and chronic illness. In the event an RN from a physician practice works in an inpatient area, the RN will have the capability to write verbal orders, enter orders, and pend access notes, and will be required to attend physician THC training. Job Responsibilities Plans care for participants and family based on assessment, standards of care and optimal outcomes. Initiates and individualises appropriate patient care guidelines, POC or clinical pathways. Maintains current plan of care by updating as needed. Develops both short- and long-term goals. Participates as member of Interdisciplinary Team by identifying caregiver and participant needs, recommending interventions and collaborating with other health care team members for mutually determined solutions. Performs technical skills according to hospital's and department's policies/procedures, appropriate competencies, and accepted standards of practice. Implements POC. Prioritises workload according to participants’ needs and requests. Bases interventions on clinical data, desired outcomes, the nursing process, and standards of care. Documents interventions and outcomes. Utilises critical thinking skills in daily care including accurately receiving, interpreting and executing physician orders. Evaluates the plan of care based on optimal patient outcomes. Documents the participant / family response to care including teaching. Collaborates with other members of the health care team to revise plan of care as needed. Supervises the care that was delegated to other health care team members. Demonstrates organisational responsibilities: identifies areas for self‑improvement, functions in relief charge / resource role as requested, completes assignments within scheduled time‑frames, cooperates with instructors to facilitate effective learning experiences for students, maintains current knowledge in area of practice, demonstrates knowledge of organisational and unit changes. Conducts and documents comprehensive patient assessments with emphasis on promotion of health, prevention and early detection in order to plan and implement care directed toward optimal specific outcomes. Includes health counselling and teaching needs in assessments. Communicates assessment findings to other health care providers as appropriate.
