Provides effective instruction to the patient and family through standardized educational programs and/or individualized training. Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes. Productive and efficient in daily operations. Selects and performs the appropriate assessment procedures. Establishes and updates formal plan of care to address individual patient needs.Required license must be valid in state of practice.**Education*** BLD – Bachelor’s Level Degree – Physical Therapy**Certification/Licensure*** Physical Therapist License (PT) – Virginia* Basic Life Support (BLS) - Certification - American Heart Association (AHA)**Experience*** Previous physical therapy experience (preferred)**Sentara Therapy Center Century** offers a comprehensive range of therapies to assist you with all of your rehabilitation needs.
