Mobile Integrated Healthcare is the provision of healthcare using patient-centered, mobile resources in the out-of-hospital environment (American College of Emergency Physicians, 2020). At Henry Ford Health System Mobile Integrated Health is an integrated service designed to enhance a patient's experience by providing comprehensive, safe and efficient in-home care as an alternative to an emergency department visit or hospitalization. The Mobile Integrated Health Medic for Population Health Management is an interdependent member of the care team who is responsible for performing comprehensive assessments, diagnostic testing and medical treatments under the direction of a physician in a patient's home or current residence. EDUCATION AND EXPERIENCE:

3-5 years of clinical paramedic experience (Community Paramedic background preferred) Demonstrated expertise in emergency medicine and chronic disease management Comprehensive knowledge of pharmacology, anatomy, and pathophysiology with ability to apply clinical pathways to high-risk populations Deep understanding of community resources and cultural competency in serving diverse populations Exceptional interpersonal and customer service skills with ability to build rapport with patients and interdisciplinary care teams Strong analytical and critical thinking abilities with resourceful problem-solving approach Excellent organizational and planning skills with capacity to manage multiple priorities effectively Superior written and verbal communication skills with meticulous documentation practices Self-directed professional who thrives working independently while remaining adaptable to evolving care environments

CERTIFICATIONS AND LICENSURES REQUIRED:

Current State of Michigan Paramedic License American Heart Association Basic Life Support (BLS) Certification American Heart Association Advanced Cardiovascular Life Support (ACLS) Certification Valid State of Michigan Driver's License

PHYSICAL DEMANDS AND WORKING CONDITIONS:

Ability to work in patient care environments with exposure to communicable diseases Manual dexterity required for clinical assessment and treatment delivery Capability to walk, stand, kneel, and stoop; occasional lifting of patients and equipment (35-50 lbs. on wheeled equipment 2-3 times per shift) Capacity to carry medical equipment (approximately 25 lbs.) into homes with stairs Comfortable driving on surface streets and highways in variable weather conditions #J-18808-Ljbffr Henry Ford Health
