The incumbent in this position is responsible for providing quality care to STOF's patients through implementing and evaluating individual patient care plans, discharge planning and complex outpatient needs. The individual performs prospective, concurrent and retrospective evaluation of clinical information to establish medical necessity, appropriateness of services and cost effectiveness and serves as patient's and family's advocate during hospital stay and outpatient status. The incumbent in this position will also be required in some circumstances to complete an in-home or at hospital evaluation of patient, if deemed appropriate. Benefits:
