How You'll Make an Impact in This Role
  • Design customized, end-to-end care plans from admission to discharge, blending clinical data with patient and family values to ensure a highly personalized healing journey.
  • Advocate fiercely for your patients by breaking down systemic barriers, optimizing insurance benefits with payers, and securing the vital resources they need for a successful recovery.
  • Evaluate medical necessity and appropriate levels of care continuously, utilizing your clinical expertise to ensure hospital resources are used effectively and efficiently.
  • Bridge the gap between the hospital and the home, serving as the primary liaison with post-acute providers to guarantee a safe, confident, and seamless transition for the patient.
  • Collaborate with the care transition team and nursing leadership, maintaining an ongoing dialogue to quickly adapt and optimize care plans as the patient's needs evolve.
What Minimum Requirements You'll Need

Licensure / Certification / Registration:

  • Registered Nurse obtained prior to hire date or job transfer date required.
  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
What Additional Preferences We're Seeking

Bachelors in Nursing, prefer at least 1 year of CM experience
