About This Role The RN Case Manager at Cleveland Clinic Tradition Hospital collaborates with inter‑professional teams to facilitate effective transitional care coordination. Highlights Location : Port Saint Lucie, FL Facility : Tradition Hospital Schedule : Full-time, Day shift What You'll Do Ensure compliance with standards of accrediting bodies and state mandates. Promote interprofessional synchronization throughout the acute episode. Complete effective handoffs to the next level of care or community. Proactively assess and mitigate risks to patients’ transitions of care. Work with patients and care partners to develop individualized transitional care plans. Participate and lead in patient rounding activities, huddles, and committee meetings. Requirements Education Graduate from an accredited school of professional nursing (Diploma, ADN, or BSN program) Experience Three years of recent RN clinical experience Two years of recent experience in utilization review, quality, or care management preferred Licenses & Certifications Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) certification required Certification by an approved entity within one year of eligibility (CTM, ACM, or CCM) preferred About Cleveland Clinic Cleveland Clinic is a leading healthcare provider committed to delivering exceptional patient care.
