The role involves managing patient care from admission through discharge, coordinating continuity of patient care with the Interdisciplinary Team, and executing complex discharge planning.

Responsibilities:

Coordinate patient care and discharge planning.
Collaborate with the Interdisciplinary Team to maintain continuity of care.

Requirements:

Experience with Midas.
Experience with Meditech.
