Work Environment This full-time position is fully remote, allowing for flexibility in your work schedule. You will have the opportunity to engage with patients primarily via telehealth methods, ensuring you can provide care to those who may have mobility challenges or live in remote areas. Your work will involve direct communication with interdisciplinary teams to coordinate care and support for our patients. Key Responsibilities Conduct comprehensive psychosocial assessments to evaluate emotional, social, and financial needs, enabling a holistic approach to patient care. Develop and implement tailored individual and group counseling sessions, focusing on issues such as adjustment to illness, grief, and coping strategies. Collaborate closely with medical teams to create cohesive treatment plans that address both psychological and physical health needs. Act as a liaison with community resources and organizations for post-discharge services, promoting continuity of care for patients transitioning back to home or community settings. Provide emotional support and education for patients and families regarding available treatment options and community resources. Participate in hospital committees, contributing social work expertise to enhance organizational practices and policies. Proactively engage in crisis intervention and emotional support, utilizing best practices and techniques to aid patients and their families during challenging times. Maintain meticulous and timely documentation in compliance with hospital policies and legal standards, ensuring accuracy in patient records and treatment plans. Patient Population / Client Focus The patients you will serve at Valley Point Healthcare range from individuals facing chronic illnesses to those in acute distress requiring immediate psychosocial support. Our facility emphasizes a patient-centered approach, recognizing the importance of mental and emotional well-being in achieving optimal health outcomes. Qualifications A Master's degree in Social Work from an accredited program is required. Possession of an active state license as a Licensed Clinical Social Worker (LCSW). Preferred Experience At least two years of experience working in a healthcare or hospital setting is preferred, providing insight into the unique challenges faced in medical environments. A strong understanding of medical terminology, mental health disorders, and crisis intervention strategies will enhance your effectiveness in this role. Excellent communication, interpersonal, and collaboration skills are essential for working effectively within multidisciplinary teams. Compensation and Benefits We offer a competitive salary range of USD $70,000 – $100,000 annually, commensurate with experience and expertise.
