Support individuals in developing and maintaining independent living skills and personal responsibility. Assist with medication routines in accordance with agency policies and procedures. Encourage participation in community, recreational, educational, and social activities. Help individuals develop meaningful relationships and community connections. Provide guidance, supervision, and positive role modeling to build confidence, independence, and life skills. Monitor for signs of crisis or behavioral concerns and respond appropriately using approved interventions. Help maintain a safe, clean, and supportive residential environment. Observe and report changes in physical, emotional, or behavioral health. Complete accurate and timely documentation in accordance with agency standards, HIPAA requirements, and regulatory guidelines. Collaborate with supervisors, nurses, clinicians, family members, and interdisciplinary team members. Participate in team meetings, ongoing training, and professional development opportunities.
