Hampton-Newport News Community Services Board is seeking a full-time Substance Use Disorder (SUD) Case Manager for Project Link in Hampton, VA. This position is ideal for social workers, counselors, case managers, and other human services professionals interested in supporting women, children, and families affected by substance use and co-occurring disorders.Our Organization Since 1971, the Hampton-Newport News Community Services Board has provided the highest quality of behavioral health services to residents of the cities of Hampton and Newport News, Virginia, offering a comprehensive array of programs for both children and adults living with mental illness, developmental disabilities, and/or substance use disorders.Your day as a Case Manager for Project LinkAs a Case Manager for Project Link, you will provide case management services to women experiencing substance use and co-occurring disorders, along with their children from birth through age seven who may be at risk for emotional or behavioral challenges. Using a person-centered, trauma-informed, and holistic approach, you will help families build stability, improve wellness, and access services that support long-term success.Responsibilities include: Conduct outreach, assess individual needs, and eligibility for servicesDevelop treatment plans and monitor progressProvide case management, crisis intervention, and supportive counselingCoordinate behavioral health, medical, housing, and community servicesConnect individuals and families with resources and supportsAdvocate for individuals and collaborate with providers and community partnersComplete documentation, case notes, and reportsParticipate in individual review meetings, discharge evaluations, and planningConduct on-site drug screenings for individuals with substance use disordersRequirementsBachelor ’s degree in Human Services (Social Work, Psychology, Sociology or Counseling) One (1) year experience providing case management services to individuals with substance use disorders or co-occurring mental health and substance use disorderValid Virginia Driver's LicenseOur BenefitsMedical, Dental, and Vision coverageLife InsuranceFlexible Spending Account (FSA)Paid Time Off (PTO)11 paid holidaysParticipation in the Virginia Retirement System (VRS)Get Started with Our team!If you are ready to bring your passion and expertise to a role where everyday matters, the Hampton-Newport News Community Services Board is the perfect place to grow your career while transforming lives.
