Registered Nurse - Hospice

We are seeking a Registered Nurse Case Manager to deliver comprehensive hospice care by developing and implementing individualized Plans of Care. Responsibilities include conducting initial evaluations within 48 hours of referral, regularly reassessing patient needs at least every 15 days, and coordinating care with patients, families, physicians, and the interdisciplinary team. Key duties involve documenting patient progress, facilitating education on self-care techniques, and ensuring quality and compassionate care in diverse home environments. Applicants must hold an unrestricted RN license in the practice state, possess a valid driver’s license, and have access to reliable transportation. Preferred qualifications include at least one year of clinical experience and CPR certification or the ability to obtain it within 90 days.
