Elite Care Management- Indiana office, is seeking a dynamic RN Floater / Case Manager to join our team. This role is ideal for a nurse who thrives in a flexible environment, enjoys variety, and is looking to expand both clinical and leadership skills .

Position Overview

As an RN Floater / Case Manager, you will be cross-trained across multiple patient cases, providing both hands-on skilled nursing care and case management oversight . This includes supporting coverage needs while ensuring high-quality, compliant patient care.

Pay Range: $83,000 - $85,000

Key Responsibilities

Perform supervisory visits and routine skilled nursing visits
Provide shift coverage as needed across assigned cases
Manage and coordinate patient care plans in collaboration with physicians and interdisciplinary team
Monitor patient progress and update care plans accordingly
Ensure compliance with all clinical, documentation, and regulatory standards
Serve as a clinical resource for field staff and caregivers

Requirements

Active RN License (State of Indiana)
Current CPR Certification
Home health or case management experience preferred
Strong clinical assessment and critical thinking skills
Ability to adapt quickly and manage multiple patient assignments
Oasis documentation preferred
