Hospice - Nurse Practitioner - Capital District, NY

The Community Hospice is seeking a full-time Hospice Nurse Practitioner. This is an exciting opportunity to join a respected and compassionate team of caregivers as it continues to provide service to the Capital District and surrounding counties. The Community Hospice is a not-for-profit 501(c)(3) organization that serves nine counties in the Capital Region of upstate New York through three regional offices and is accredited by the Community Health Accreditation Program (CHAP). We are part of St. Peter's Health Partners and are privileged to work with every hospital in the region to provide hospice services. We also collaborate with 43 skilled nursing facilities and many assistive living/adult homes across our service area. The majority of our patients are served where they prefer to be at end of life to maximize quality of life - their home. We have an average daily census of over 700 patients. Our service area covers: Albany, Schenectady, Rensselaer, Saratoga, Columbia, Greene, Schoharie, Washington and Montgomery Counties. Position Details: Provide oversight and medical consultation for hospice patients including establishing appropriate pain and symptom management, provision of Advance Care Planning discussions, and assisting with discharge dispositions as appropriate.
