Medical Social Worker

Schedule: 5 days a week, 8:00 AM – 5:00 PM. No after‑hour on‑call requirements. Good Samaritan Hospice Indiana – Illinois is hiring a full‑time Medical Social Worker. Job Summary and Responsibilities As a Medical Social Worker , you’ll engage in a range of responsibilities, including: Establishing a care plan in accordance with the physician’s plan of treatment. Providing crisis intervention as required. Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically. Providing patient care education to patients and caregivers. Assisting in making appropriate community referrals to gain access to additional resources (e.g., applications for public assistance, housing problems, additional health care resources, transportation). Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship. Completing and submitting all required documentation in an accurate and timely manner. Service Areas Southwestern Indiana: counties of Davies, Gibson, Greene, Knox, Martin, Pike, and Sullivan. Illinois: counties of Lawrence, Wabash, Crawford, and Richland. Job Requirements Master’s degree from an accredited school approved by the Council on Social Work Education, and certification under IC25‑23.6‑4‑3 in Indiana. Currently licensed to practice in the state served. One year of social work experience in a health care setting.
