Provider Qualifications Note: You can apply if you are a Board Certified provider or truly Board Eligible. You need to be certified in your specialty, or board eligible and become certified within 2 years of being board eligible. Qualifications Required Must be board certified in gastroenterology, or board eligible with a plan to get certified within 2 years – this is a must (no exceptions). Must have a valid Michigan license when starting the job. Must have a DEA license. Must have BLS Certification and keep it updated. Certification Requirements BLS Board Certified Board Eligible State License Requirements License to practice in Michigan.
