Therapist - Speech Language Pathologist

Job Summary:
Provide specialized speech-language pathology services to patients in a long-term care setting, focusing on assessment, diagnosis, and treatment of communication and swallowing disorders. Collaborate with multidisciplinary teams to optimize patient outcomes and ensure quality care.
Responsibilities:
- Assess patients’ speech, language, and swallowing functions to determine therapy needs.
- Develop and implement individualized treatment plans based on clinical evaluations.
- Document patient progress and adjust therapy strategies accordingly using EMR software.
- Communicate effectively with patients, families, and healthcare staff regarding treatment goals and progress.
- Adhere to regulatory, licensing, and certification requirements, including maintaining NYS licensure and ASHA certification.
- Maintain accurate and timely records in compliance with facility and reimbursement guidelines.
Qualifications:
- Hold a valid NYS license as a Speech-Language Pathologist.
- Possess a Certificate of Clinical Competence in Speech Language Pathology from ASHA or eligibility.
- Demonstrate excellent assessment and communication skills.
- Have LTC experience and familiarity with PDPM reimbursement systems.
- Proficiency in MS Office and Epic EMR software.
- Willingness to travel within a 75-mile radius or local area as needed.
