American Medical Home Health Services is seeking a dependable and experienced Registered Nurse (RN) to provide home health visits in Northwest San Antonio and surrounding areas. Position Details PRN/per-visit position Flexible scheduling Competitive per-visit rates Patient assignments primarily in Northwest San Antonio Electronic documentation through WellSky/Kinnser NestMed AI available to assist with clinical documentation Responsibilities Complete comprehensive patient assessments Perform skilled nursing visits according to the plan of care Complete Start of Care, Resumption of Care, Recertification, and Discharge assessments, Supervisory, Routine visits as assigned Communicate changes in the patient’s condition to physicians and the clinical team Provide patient and caregiver education Submit accurate and timely clinical documentation Coordinate care with therapists, providers, and other members of the interdisciplinary team Qualifications Current and unrestricted Texas RN license Minimum of one year of nursing experience Home health and OASIS experience strongly preferred Current CPR certification Reliable transportation, valid driver’s license, and automobile insurance Ability to manage visits independently and complete documentation promptly Join a supportive home health team while enjoying flexible scheduling and competitive per-visit compensation.
