Social Worker

Job Summary

RYZE on the Avenue is seeking a compassionate and dedicated Social Worker to join our team. The Social Worker will provide support to residents and their families by addressing psychosocial needs, assisting with care planning and discharge planning, advocating for resident rights, and connecting residents with appropriate resources and services. The ideal candidate will have strong communication and interpersonal skills and a passion for helping residents maintain their dignity, independence, and quality of life.
Responsibilities

• Provide social services support to residents and their families.
• Assess residents' psychosocial needs and assist in developing individualized care plans.
• Provide emotional support, counseling, and crisis intervention as needed.
• Participate in interdisciplinary care plan meetings and collaborate with nursing and other departments.
• Assist residents and families with adjustment to the facility and changes in health or living circumstances.
• Coordinate discharge planning and assist residents with transitioning to the appropriate level of care.
• Connect residents and families with community resources, financial assistance programs, healthcare services, and other support services.
• Advocate for resident rights, dignity, privacy, independence, and quality of life.
• Assist residents with advance directives, guardianship concerns, and other social service needs.
• Respond to resident and family concerns and work with facility leadership to resolve issues appropriately.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Assist with admissions, transfers, and discharges as needed.
• Develop and maintain positive relationships with residents, families, community organizations, hospitals, and other referral resources.
• Participate in facility meetings, quality assurance activities, and other interdisciplinary initiatives.
• Maintain compliance with applicable federal, state, and local regulations, including CMS and IDPH requirements.
• Perform other duties as assigned by facility leadership.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field required.
• Social Work licensure or certification in Illinois preferred.
• Previous experience in a skilled nursing facility, long-term care, healthcare, or social services setting preferred.
• Strong knowledge of psychosocial services, discharge planning, resident rights, and community resources.
• Excellent verbal and written communication skills.
• Strong interpersonal, organizational, and problem-solving skills.
• Compassionate, professional, and resident-focused approach.
• Ability to work effectively with residents, families, and interdisciplinary teams.
• Ability to maintain confidentiality and handle sensitive situations professionally.
• Proficiency with Microsoft Office and electronic medical record systems preferred.
