This role involves conducting patient evaluations, creating individualized treatment plans, and delivering direct therapy services aimed at restoring function and mobility.Responsibilities:\n \n Screen, evaluate, and treat patients effectively.\n Develop and maintain updated treatment and discharge plans of care.\n Document services accurately and in a timely manner.\n Supervise Physical Therapy Assistants (PTAs) and Rehabilitation Technicians as per practice acts.\n Collaborate with interdisciplinary care teams and engage with families.\n Ensure compliance with safety, infection control, and regulatory standards.\n Qualifications:\n \n Graduate of an APTA-accredited Physical Therapy program (DPT/MSPT/BSPT).\n Active state Physical Therapist license or the ability to obtain one.\n Strong communication, documentation, and clinical skills.\n Flexible and adaptable to varying facilities, schedules, and patient needs.\n \n \n Wellspring Nurse Source Job ID #37554922.
