**Responsibilities:**

* Provide direct patient care to clients in their homes, including assistance with activities of daily living (ADLs), grooming, and hygiene
* Monitor and record vital signs, including blood pressure, temperature, and pulse
* Assist with medication administration and management
* Maintain accurate and detailed records of client care and progress
* Collaborate with other healthcare professionals to develop and implement comprehensive care plans
* Demonstrate a commitment to providing patient-centered care that respects the unique needs and preferences of each client
