Responsibilities:
- Perform social work assessments and interventions for patients in an acute care setting.
- Coordinate discharge planning and facilitate transitions to appropriate levels of care.
- Collaborate with interdisciplinary teams to support patient care and discharge goals.
- Provide counseling and resource linkage for patients and families.
- Document social work activities and maintain accurate patient records.
- Assist with addressing psychosocial barriers to care and discharge.
- Participate in departmental meetings and quality improvement initiatives.
- Master's degree in Social Work (MSW) with relevant licensure or certification as required.
- Minimum 2 years of acute medical social work experience including discharge planning.
- Strong assessment, counseling, and case management skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within interdisciplinary teams.
- Proficiency in documentation and electronic medical record systems.
- Ability to manage a caseload with a ratio of approximately 1:25 during day shifts.
- Experience working in acute care hospital settings.
- Knowledge of community resources and healthcare systems.
- Familiarity with discharge planning regulations and best practices.
- Typically required to work one full weekend every four weeks, subject to department needs.
- Floating to other units is not expected.
- Return to Office (RTO) restrictions limit work in the facility to no more than 7 consecutive days.
- Professional attire includes business attire or scrubs as appropriate.
- First-time travelers must undergo screening before assignment.
- Day shift schedule with patient ratio approximately 1:25.
- Weekend coverage typically one full weekend per four-week period.
- Shifts and assignments determined based on department needs and patient census.
