Facility: Spring Creek Rehabilitation & Nursing Care Center Location: Brooklyn, NY Position: Home Health Aide (HHA) Schedule: PRN Pay: $19–$20 per hour

Spring Creek Rehabilitation & Nursing Care Center is seeking a dependable and compassionate Home Health Aide (HHA) to assist residents with safe transportation to clinic and dialysis appointments and provide support throughout their visits.
Responsibilities
- Escort residents safely to and from clinic and dialysis appointments.
- Arrive approximately 1 hour before scheduled appointments.
- Assist residents with preparing for appointments and provide approved snacks for out-on-pass visits.
- Monitor residents for safety and help prevent falls or incidents.
- Report accidents, incidents, or concerns promptly to the Clinical Coordinator or ADNS.
- Ensure required medical records and paperwork accompany residents.
- Verify DNR/DNI status and required documentation.
- Communicate clinic concerns and advocate for residents.
- Coordinate assignments and appointments with the Clinic Coordinator.
- Follow all facility policies, safety procedures, and abuse-prevention regulations.
- May be required to attend multiple appointments in one day.
Requirements
- Current NYS HHA certification required.
- CNA certification preferred.
- High school diploma or equivalent preferred.
- Previous nursing home or healthcare experience preferred.
- Strong communication and interpersonal skills.
- Professional appearance and dependable work ethic.
- Ability to prioritize resident safety and follow instructions.
