Join River Oak's Full-Service Partnership Wraparound (FSPW) team in Elk Grove and help children, youth, and families build on their strengths and succeed at home, school, and in the community.As part of our certified High Fidelity Wraparound program, you'll work with a collaborative team to provide flexible, family-centered mental health services and support.In this role, you will:Support infants, children, and youth ages 0–21 in your community.Help youth and families achieve their goals and navigate the systems that support them.Promote success at home, school, and in the community.Provide flexible services based on each family's needs, including varied locations and schedules.Work with families, treatment teams, and community partners to develop individualized, strength-based care plans.At River Oak, equity, diversity, and inclusion are part of our people's strategy which reflects our vision of building an organization that attracts and leverages diversity in our staff, and strives for respect and inclusion throughout our workplace and in the communities we serve.Program Summary:The River Oak Full-Service Partnership Wraparound (FSPW) program provides intensive, individualized, family-centered mental health services and support to children and youth with complex behavioral and emotional needs who are involved in or at risk of entering the juvenile justice system, experiencing placement instability, or transitioning from residential care to a family setting. Through a collaborative, non-traditional team approach, FSPW partners with youth and their families to build on their strengths and develop comprehensive service plans that may include evidence-based interventions, care coordination, and natural community supports. Services are flexible and accessible, delivered in homes, schools, clinical sites, and community settings to meet each family's unique needs. The program aims to promote stability, strengthen family relationships, support academic and vocational success, reduce homelessness, psychiatric hospitalizations, and involvement in detention or incarceration, and help youth remain safely in their homes, schools, and communities.Job Responsibilities:Coordinate individualized care with youth, families, treatment teams, and community partners.Develop, review, and coordinate care plans and client goals.Lead Child and Family Team (CFT) meetings and coordinate services.Conduct home, school, office, and community visits for assessment and support.Provide individual and family support to build strengths and achieve goals.Provide crisis support and participate in on-call duties as required.Coordinate resources, advocacy, referrals, and community services.Meet billing goals and track productivity.Maintain accurate online records while ensuring confidentiality and HIPAA compliance.Demonstrate effective communication, empathy, and professionalism in client interactions.Collaborate with multidisciplinary teams and community partners.Stay updated on professional standards, cultural awareness, and available resources.Proficient in Microsoft Office and electronic documentation systems.Maintain strong organizational, time management, and problem-solving skills.Minimum Qualifications:Possess a masters degree from an accredited university or college with course of work in social work, psychology, and counseling or a closely related field.Two (2) years' full-time experience in providing mental health services/social services case management or mental health counseling; services must be provided in mental health, community-based, or probation setting.Possess a bachelor's degree from an accredited university or college with course of work in social work, psychology, and counseling or a closely related field.Four (4) years' full-time experience in providing mental health services/social services case management or mental health counseling; services must be provided in mental health, community-based, or probation setting.Possess an associate's degree from an accredited university or college with major course work in social work, psychology, and counseling or a closely related field.A minimum of six (6) years' full-time experience in providing mental health services/social services, case management or mental health counseling; services must be provided in mental health, community-based, or probation setting.Please note: This position does not provide supervision for candidates that are looking to receive their BBS Registration/Licensure.Benefits:Medical, dental, and vision plansEmployee Assistance Program (EAP)Life InsuranceTuition AssistanceGenerous paid Vacation and Sick Leave12 Paid HolidaysFlexible Spending Accounts (FSA)Retirement Savings Plan403(b) Retirement PlanShort and Long term Disability InsuranceIndividual and group supervision in accordance with the rules and regulations of the Board of Behavioral Science (BBS) and Board of Psychology (BOP).**Culturally diverse and/or bilingual candidates are encouraged to apply**
