Position Summary

The Certified Nursing Assistant (CNA) is an essential member of the interdisciplinary care team and works under the direction and supervision of a Registered Nurse (RN) and/or Licensed Practical Nurse (LPN). The CNA provides direct care and assistance with activities of daily living to residents in accordance with each resident's individualized plan of care, facility policies and procedures, and applicable state and federal requirements.

The CNA promotes each resident's dignity, independence, comfort, safety, and quality of life while providing compassionate, resident-centered care. This position requires close observation of residents, timely communication of changes in condition, effective teamwork, and a willingness to assist residents and coworkers as needed.

The CNA is expected to follow infection prevention and safety practices, maintain resident privacy and confidentiality, respect individual preferences and cultural differences, and communicate professionally with residents, families, visitors, and members of the interdisciplinary team.

Essential Duties and Responsibilities

• Provide direct resident care in accordance with the resident's individualized plan of care and instructions from the RN/LPN.
• Assist residents with activities of daily living (ADLs), including bathing, grooming, dressing, oral care, toileting, eating, mobility, and other personal care needs.
• Assist residents with transfers, ambulation, turning, positioning, and repositioning using proper body mechanics and appropriate assistive or mechanical lift equipment.
• Assist residents with meals, including meal setup, feeding assistance, and providing fresh drinking water in accordance with resident needs and prescribed dietary or swallowing precautions.
• Obtain and accurately record vital signs, weights, intake and output, and other resident information as assigned and within the CNA's scope of practice.
• Observe residents for physical, emotional, behavioral, or functional changes and promptly report changes or concerns to the RN/LPN.
  Respond promptly and courteously to resident requests and call lights.
• Assist with admission, transfer, discharge, and transportation of residents as directed.
• Promote resident independence by encouraging residents to fully participate in their care and activities as much as possible.
• Provide care in a manner that protects residents' dignity, privacy, personal preferences, and rights.
• Follow standard precautions, infection prevention practices, isolation procedures, and appropriate use of personal protective equipment (PPE).
• Maintain a clean, safe, comfortable, and orderly resident environment.
• Make and change beds and assist with maintaining resident rooms and common care areas.
• Collect specimens and perform other resident-care tasks as permitted by training, competency, facility policy, and applicable regulations.
• Perform blood glucose testing when appropriately trained, authorized, and within the CNA's permitted scope of practice, and promptly communicate results to the RN/LPN.
• Accurately document care provided, observations, and other required information in the electronic health record or other designated documentation system.
• Communicate effectively with nursing staff regarding resident needs, concerns, preferences, and changes in condition.
• Work collaboratively with nursing, therapy, dietary, activities, social services, environmental services, and other departments to support resident care and quality of life.
• Maintain adequate supplies and equipment in assigned areas and notify the appropriate individual when supplies or equipment are needed.
• Follow facility emergency procedures and respond appropriately during emergency situations.
• Participate in required orientation, in-service education, competency validation, and other training programs.
• Maintain required certifications and competencies.
• Support new employees, students, and other team members as appropriate.
• Immediately report suspected abuse, neglect, exploitation, mistreatment, injuries of unknown origin, or misappropriation of resident property in accordance with facility policy and applicable requirements.
• Maintain confidentiality of resident and facility information in accordance with HIPAA and facility policies.
• Demonstrate professional, respectful, and compassionate behavior in all interactions with residents, families, visitors, and coworkers.
• Perform other duties related to resident care and facility operations as assigned and within the individual's training and permitted scope of practice.
