Home Care Coordinator to join our growing team. This hybrid role combines administrative leadership, client relationship management, and caregiver support, ensuring smooth day-to-day operations and quality care for our clients. The role requires you to have experience in home care or healthcare administration, a heart for caregiving, and strong leadership skills. 

Key Responsibilities: Scheduling & Staff Coordination 

• Manage daily and weekly caregiver schedules using agency software. 
• Match caregivers with clients based on skills, availability, and compatibility. 
• Monitor attendance, respond to call-outs, and arrange coverage quickly. 
• Ensure compliance with shift documentation and timekeeping. 

Customer Service & Client Relations 

• Serve as the first point of contact for client inquiries and concerns. 
• Provide excellent customer service through phone, email, and in-person communication. 
• Build and maintain strong, trusting relationships with clients and their families. 
• Conduct regular client check-ins to assess satisfaction and service needs. 

New Client Admissions 

• Coordinate and conduct initial in-home assessments and service plan development along side of Admissions. 
• Gather intake documentation and ensure timely onboarding. 
• Communicate clearly with clients and families about services and expectations. 
• Collaborate with staff and caregivers to launch services efficiently with proper introductions and orientation. 
• Provide proper introduction for any and all new client / Caregiver arrangement are needed. 

Leadership & Team Support 

• Lead with empathy and accountability; motivate and support caregivers in the field. 
• Assist with onboarding and training new staff. 
• Promote a positive, respectful workplace culture. 
• Participate in team meetings and quality assurance initiatives. 

Caregiving (As Needed) see addtional caregiver job description 

• Provide in-home care to clients during emergencies, staff shortages, or training purposes. Cover shifts when call outs. 
• Assist clients with ADLs (Activities of Daily Living), companionship, and safety supervision. 
• Document services provided in line with agency policies. 

Qualifications: 

• Minimum 1 years in home care administration, healthcare coordination, or scheduling. 
• CNA, HHA, or caregiver experience preferred 
• Strong organizational, multitasking, and problem-solving abilities. 
• Proficient in scheduling and communication software. 
• Excellent interpersonal and leadership skills. 
• Valid driver’s license and reliable transportation. 

Work Environment: 

• Office/Home base visits and care as needed. 
• On-call responsibilities apply
